CHAMPION TAEKWONDO CENTER, INC.          Release and Waiver of Liability for Summer Camp

 

In consideration of being permitted to participate in any manner at Champion Taekwondo Center, Inc., summer camps, I/The Minor, for myself, my personal representative, heirs, and next of kin:

 

I/The Minor fully understand the nature of Taekwondo/Martial Arts and acknowledge that my/the minor’s experience and capabilities and believe I/the minor am qualified to participate in such practices, events, and demonstrations. I understand the facility for which practices, events, and demonstrations is open to the public.  I further agree and warrant that if at any time I believe conditions to be unsafe, I will immediately discontinue further participation in practices, events, and demonstrations.

 

I/THE MINOR FULLY UNDERSTAND THAT TAEKWONDO/MARTIAL ARTS INVOLVES RISKS AND DANGERS OF SERIOUS BODILY INJURY, INCLUDING PERMANENT DISABILITY, PARALYSIS AND DEATH (“RISKS”).   These risks and dangers may be caused by my/or the minors own action or inactions, the actions or inactions of others participating in the Taekwondo/Martial Arts conditions in which the practice, event, and demonstration takes place, THE NEGLIGENCE OF THE RELEASES NAMED BELOW.  There may be other risks and foreseeable at this time; and I/the minor fully accept and assume all such risks and all responsibility for losses, costs and damages I incur as a result of my participation in Taekwondo/Martial Arts practices, events, and demonstrations related to Champion Taekwondo Center, Inc.  I acknowledge that Taekwondo/Martial Arts is a contact sport and accept all the inherent risks involved.

 

I/the minor hereby release, discharge, and covenant not to sue, and agree to hold harmless: Champion Taekwondo Center, Inc., all instructors and students of the school, their parents/relatives, volunteers, and associates.  I WILL ENDEMNIFY, SAVE AND HOLD HARMLESS EACH OF THE RESEASEES FROM LITIGATION EXPENSES, ATTORNEY FEES, AND LOSS LIABILITY, DAMAGE OR COST AS MAY INCUR AS THE RESULT OF ANY SUCH CLAIM. 

 

I/the minor understand that pictures and articles taken or written with my image or my name and put on the official web site, and I/the minor understand that those pictures and articles are the property of Champion Taekwondo Center, Inc. to be used for their official use as a Martial Arts school to promote and advertise the school as they deem necessary.  I/the minor expect no compensation for the use of my image or name.

 

I HAVE READ THIS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I/THE MINOR HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT AND HAVE SIGNED IT FREELY AND WITHOUT ANY INDUCEMENT OR ASSURANCE OF ANY NATURE AND INTEND IT TO BE A COMPLETE AND UNCONDITIONAL RELEASE OF ALL LIABILITY TO THE GREATEST EXTEND ALLOWED BY THE LAW AND AGREE THAT IF ANY PORTION OF THIS AGREEMENT IS HELD TO BE INVALID THAT THE BALANCE, NOTWITHSTANDING, SHALL CONTINUE IN FULL FORCE AND EFFECT.
